How the score is built. Each level maps to a point weight derived from published hazard ratios for hard outcomes (1 point ≈ 20% relative hazard; ~4 points ≈ doubling). System sum is capped to prevent correlated axes dominating; end-organ damage carries the heaviest weight and can override the numeric stage. All weights sit in an editable config layer and are provisional until data-validation (Paper 2).
